The Mission Of Pathways To Housing Dc Is To End Homelessness For The Most Vulnerable Homeless Individuals: Persons Who Are Chronically Homeless - Cycling Between The Streets, Shelters, Jail Cells And Emergency Rooms- And Living With Serious Psychiatric Disabilities And Substance Use Issues. Once Pathways Identifies A Person On The Street Who Fits This Description, They Provide Them With An Apartment Of Their Own, As An Immediate And Practical Solution To Their Homelessness, Without First Requiring Compliance With Psychiatric Or Substance Abuse Treatment. Simultaneously, The Program Provides The Intensive Wraparound Services To Address The Individuals? Multiple Needs, Which Include Mental Health And Addictions Treatment, Supported Employment Services, Assistance With Entitlements, Skills Building To Live Independently, And Reconnections To Family And Community. Assistance Continues To Be Provided Every Step Of The Way, 24/7, So Tenants Have All The Support Necessary To Move Into And Integrate Into Their Community, And To Begin The Long Journey Through The Recovery And Rehabilitation Process.
